We are looking for a Case Manager – Mental Health Professional (MHP) who exemplifies that mission, and who wants to work for an organization that makes a difference. Are you the right fit?
What will you be doing?
- Providing counseling and case management services to a caseload of approximately 30 – 50 clients with diagnoses of persistent mental illness and/or substance abuse
- Developing and implementing individual programs to maximize the clients’ independent community living skills
- Completing daily documentation of mental health services provided to clients
- Working with a team of behavioral health service providers within the agency to ensure a positive client-centered approach
What will you bring to the table?
- Bachelor’s degree in human services
- Experience working in social services (required)
- A valid Illinois driver’s license and automobile insurance

What are the other requirements?
- Navigation of stairs
- Ability to lift 50 lbs. or more
- Ability to provide services in a client’s home
- The use of your personal vehicle or agency vehicles to transport clients
- Maintaining required certifications and other relevant trainings
