Case Manager QIDP - Child & Family Support and Home Based Support
The Case Manager provides intake, assessment, linkage, advocacy, referral, and follow-up services to individuals presenting for admissions to all DD programs at AID. The Case Manager works with a client caseload and is responsible to collaborate with them and their families or others involved in their service, to develop plans aimed at maximizing their potential and optimizing ability to live successfully in the community.
The Case Manager also provides Self Directed Services for Home Based clients in the Elgin area.
AID facilitates services for individuals from diverse backgrounds, embraces differences in individuals and promotes a positive attitude with peers and community members.
- Bachelor’s Degree in Human Services field.
- QIDP Required
- One year of experience, working directly with individuals with intellectual disabilities and/or other developmental disabilities
